Please Note:
If your child requires an EPI Pen and/or Inhaler, you will need to have your pediatrician fill out the Individual Health Plan below and physically bring it with  your child's medication to our office. Registrations will NOT be taken in the office and online registrations are not considered complete with out the medication and proper documentation for it.

There is a $50 deposit per week due at the time of registration. The balance for weeks 1 - 5 is due by Friday, June 16th and weeks 6-8 is due by Friday, July 21st.
